Regeneron’s October 1, 2026 COURAGE report described less lean-mass loss with trevogrumab plus semaglutide than with placebo plus semaglutide. Its approximately 70% preservation headline concerned a thigh-muscle MRI subgroup, not the whole-body DXA result.[1]

- Whole-body DXA vs thigh MRI
What kind of source is this?
This note summarizes a sponsor announcement. The release described conference findings and a paper in press. It should be distinguished from a complete peer-reviewed report and checked for later publication.[1]
The source format matters because readers need the detailed methods, analysis population, missing-data approach, and safety tables to evaluate a clinical result fully.
What did whole-body DXA show?
At 52 weeks, the lower-dose portion reported DXA lean-mass changes of −7.3% with placebo plus semaglutide, −5.8% with one trevogrumab combination, and −4.2% with the other. The sponsor expressed the latter differences as 20.5% and 42.5% relative preservation.[1]
A reduction in the amount lost is different from an increase above baseline. Both treatment-group values remained negative. This is why the word “preservation” should be accompanied by the actual comparison.
What did the MRI subgroup measure?
The MRI analysis concerned thigh-muscle volume in participants with evaluable scans and reported roughly 70% relative preservation.[1] That is a different anatomical scope and measurement method from whole-body lean mass.
When two outcomes appear in one release, keep their names, participants, and units separate. Taking the larger percentage from one and attaching it to the other creates an unsupported result.
What remains important to evaluate?
Body-composition findings and performance outcomes are different. A reader should look for direct measures before making a claim about strength, independence, or daily function. The full benefit–risk interpretation also requires the safety findings.
This report concerns a particular combination-treatment setting. It is not evidence for a retail research peptide or an instruction to alter an existing treatment. Compare its methods with BELIEVE, while retaining the difference between a sponsor announcement and a full publication.
Questions, answered.
Does 70% mean participants gained 70% more muscle?
No. The headline describes relative preservation of thigh-muscle volume compared with a loss in the comparator, not a 70% increase in muscle.
Are DXA lean mass and MRI thigh volume the same measurement?
No. They differ in anatomical scope, method, and units. Their percentages should not be exchanged.
